Dr. Khatami is an in-network provider for Anthem Blue Cross and Aetna. For patients with other insurance plans, payment is due in full at time of service. Other insurance plans may have an out-of-network benefit. Please contact your insurance carrier and inquire about your out-of-network benefits and percentage of reimbursement. Dr. Khatami can provide you with an invoice to submit for insurance reimbursement.
